Consent Form - SignNow

Use the following checklist to complete, send, and document consent forms within the CRM.

Consent Form

  • Include the client’s full name in CN Consent (e.g. first name last name-CN Consent)

  • Add client information to the Client Information section

  • Add courses with program and provider for consent to register (add a page if more than 20 courses)

  • For company consent, ensure names and emails of all employees/volunteers/clients are listed and it is clear which courses each is to be registered in

Find the consent form sent email in the territorial email queue in CRM

Add the client in “Regarding” before removing from the queue

Returned Consent Form

When signed consent comes back, add the client in “Regarding” before removing them from the email queue

  • Open the consent form and verify:

    • Signed and dated

    • “Yes” checked for consent

    • All courses, programs, and providers are listed and visible (nothing missed or cut off)

    • For company consent, ensure all names/emails are listed and visible

  • Use the territorial SignNow account.
  • If covering multiple territorial teams, sign in using the appropriate territorial email as the username, so the client sees it from the correct team and the completed form goes to the correct email queue in the CRM.

  • Never change the username or password for your SignNow account.

  • Always include the Client Information - First Layer and Client Signature - Last Layer. Both are required to make the consent form valid.

  • Only select the consent options needed for the client – do not send every consent option just in case – only send the options required for that client's specific situation.

  • Fill in as much information as possible in the client information section and always list courses, programs (if applicable), and providers on a Consent to Register and Consent to Submit Application.
  • Never send a blank consent form.

  • Don’t add fields to a consent form or use consent forms as intake or information-gathering documents.

  • Make sure all required consent options have been checked “yes” before taking any action on behalf of a client.

  • Obtain consent from a parent or guardian for clients under the age of 18 (Under 18 Parental Guardian Consent Form) before taking any action or sending any other consent form.

  • Remember to add the client’s name in the “Regarding” field on the email in the CRM before removing it from the queue and adding the signed consent form to the Consent tab.
